Dear Triad Behavioral Resources Patient,

On April 28th Triad Behavioral Resources closed its downtown location at 201 N Front Street, Wilmington.

You will now receive your injection at a new location. Starting May 12, 2023, your injection appointments will be at 3333 Wrightsville Ave M, Wilmington, NC 28403 in the Azalea Coast Behavioral Health offices.

Everything else will remain the same. You will have your appointments via Telemed with Edward McKeithan, FNP-BC. Jess will be at the new location to give your injections. You will call the office to make your next appointment – 910-765-1003.
